No, I just have RSI in my index and middle fingers.

Seriously though, if you had both thumbs up and were using your fingers, you'd have to be lying... well, work it out for yourself. Put it this way, if there wasn't a lot of space it would be awkward. Besides, you might want to be kissing them. Or sucking nipples, or something. So, while two thumbs would be possible, in general it would be simpler to use fingers from one hand only.
